To apply by phone: call 612-334-5970 or 1-800-292-4150 Monday through Thursday between the hours of 9:30 AM - 12:30 PM and 1:30 - 3:30 PM, or Fridays between 9:30 - 11:45 AM. If you are unable to call during intake hours, call 612-332-1441 or 1-800-292-4150 to request assistance.
