What is the Father's Adoption Registry? 

This is a registry of men who believe themselves to be the father of a born or unborn child. A man who believes he is the father of a child may register with the Father's Adoption Registry. Registration is done through the Minnesota Department of Health. The woman named by the father will be notified of this registration.

To receive notice of an adoption, the father must register with the Father's Adoption Registry within 30 days after the birth of the child.

After an adoption petition is filed, the registered father must do one of the following within an additional 30 days:

• File to relinquish his parental rights, or
• Claim his parental rights and take affirmative steps toward establishing paternity.

These forms are for uncontested stepparent adoptions, meaning that all parties agree to the adoption. Make sure you read the form instructions to find the right forms for your case.  

The child in question in the adoption case, the child's Indian tribe, or a Social Services Agency may intervene without the Courts permission. If you are not one of these but you want to intervene in the case, you will need to fill out forms to ask for the Courts permission to intervene.
