Court Order for Child's Passport for Unmarried Mothers
Do I need my child's father to sign or give consent to get my child a passport?
If a court order for custody does not exist and there isn't a pending custody case in court, the biological mother has sole custody by state law. An unmarried mother can ask the court to issue an order that allows her to apply for a passport for her child.
You cannot use these forms if there is a court order granting custody of the child to you or any other person.
Do I need a separate form for each child?
You can use the form for multiple children as long as the children have the same father. If the children have different fathers, then you will need to use separate forms.
