Changing a birth record in Minnesota

Birth Record Amendments 

Welcome! Amending a birth record means correcting or completing information on a birth certificate. You may want to request an amendment to your child’s birth record if something is missing from their birth certificate or is incorrect on the certificate. If you click the link above, you will see forms to amend the certificate and instructions to fill out the forms.   

Amending a birth record is not the same as replacing a birth record after an adoption. The Minnesota Department of Health has separate instructions and forms for that.
