Revocation of a Power of Attorney - Do It Yourself

You will need certain information to finish the interview. This includes:

• The date you signed and Notarized the Power of Attorney you are revoking

• The name of your attorney-in-fact

• The name of the county where you will get your Revocation form Notarized
